Administered by Shamrock Rx, at Old Second Bank, Route 38 and County Line Road, Maple Park. Cash, check, Blue Cross Blue Shield of PPO Insurance, and Medicare Part B accepted. (No HMO Supplemental)
Shots are available for people 18 years and older. A physician’s note is required if you are pregnant.

To have information posted on the Village website your Activity/Event must meet the following requirements:
– Event/Activity must be located within the Village of Maple Park.
– Only 4 listings per entity / business / organization for any given month.
